Primary care networks (PCNs) in the Midlands, East of England and South East regions are invited to put forward expressions of interest (EOI) for CLEAR transformation support to develop new models of care for the prevention of cardiovascular disease (CVD).

Commissioned by NHS England, 14 PCNs – two from each NHS England region – are being offered the opportunity to take part in a five-month project to develop new models of care for people at risk of developing CVD to help them to lead longer, healthier lives.

The programme is split into two phases, phase one will run from January to May 2024, followed by phase two from May to September 2024.  There are remining places in phase two of the project for PCNs in the Midlands, East of England and South East.

The support is part of ongoing nationwide work focused on key NHS Long Term Plan ambitions and the national CVD programme. It will be based on preventative new models of care and workforce that were developed with frontline staff at three PCNs in Cheshire and Merseyside as part of CLEAR Living Well 2023.

The programme will comprise three regional workshops and a series of one-to-one meetings when PCNs will be assisted to build a business case for the new model of care. Through the workshops, the PCNs will be guided through the CLEAR approach including clinical engagement and data analysis to identify patients at risk of developing CVD and other related long-term conditions.

The deadline to send an EOI for remaining places in phase two is 5pm on Friday 16 February 2023.
